If you're seeing this message in Liberty, it is typically because you're trying to add an account that already exists. If you're not seeing the already-existing account in the Account List, make sure you un-check all of the options at the top of the Account List...

The big one is the "Hide Customer" check box. This hides any accounts that do NOT have items within them. If you've just added a bunch of accounts, but they have no items, this check will hide them from your view.
